Millway Dental respects the privacy of every patient and website visitor. This policy explains what personal information we collect, why we collect it, how it is protected, and the choices you have. It covers both this website and the personal health information we hold as your dental care provider.

Who we are

Millway Dental is a dental practice located at 3200 Erin Mills Pkwy, Mississauga, Ontario L5L 1W8. You can reach us at (905) 607-4333 or millwaydentaloffice@gmail.com.

As a dental practice in Ontario, we are a health information custodian under the Personal Health Information Protection Act, 2004 (PHIPA), and we follow the record-keeping and privacy standards set by the Royal College of Dental Surgeons of Ontario (RCDSO).

Information we collect

Through this website

The only information this website asks you for is what you type into the callback request form: your name, phone number, an optional email address, and an optional message. Name and phone number are required so we can call you back; the other two fields are yours to leave blank.

That form is emailed directly to the practice. It is not stored in a website database, and it is not sent to any advertising or marketing service.

This website does not use Google Analytics, Facebook/Meta pixels, tag managers, advertising trackers, or any similar tool, and it does not set tracking cookies of its own. WordPress, the software this site runs on, may set a small number of functional cookies (for example if someone logs in to administer the site). As with any website, our web host keeps standard server logs, which can include IP addresses, for security and troubleshooting.

As your dental care provider

When you become a patient, we collect the information needed to provide safe, appropriate care and to administer your account. This typically includes your contact details, date of birth, health and dental history, medications and allergies, clinical notes, x-rays and other diagnostic images, treatment records, and dental insurance and billing information.

How we use your information

  • To provide dental treatment and plan your care
  • To contact you about appointments, recalls and follow-up
  • To submit claims and predeterminations to your dental insurer, with your consent
  • To administer billing and payment
  • To meet our legal, regulatory and professional obligations

We do not sell your personal information, and we do not share it for advertising purposes.

When we share information

We share personal health information only where it is necessary to your care or required by law, including:

  • with other health care providers involved in your treatment, such as a specialist we refer you to, on a need-to-know basis
  • with your dental insurer or benefits administrator, where you have asked us to submit claims or predeterminations on your behalf
  • with dental laboratories fabricating your appliance or restoration
  • where disclosure is required or permitted by law, or by the RCDSO

Any other disclosure requires your consent, which you may withdraw at any time (subject to legal and record-keeping limits).

Third-party content on this website

Some pages embed content from other companies. When that content loads, those companies may receive your IP address and set their own cookies, under their own privacy policies rather than ours:

  • Google Maps — the map and the 360° tour on our contact page
  • Google Fonts — the typefaces used across the site
  • YouTube (Google) — where a video is embedded on a page

Our Facebook and Instagram icons are ordinary links. Nothing is shared with those platforms unless you click through to them.

How long we keep records

Dental records are retained for the period required by the RCDSO — at least ten years from the date of the last entry in the record, or ten years from the day a patient turned eighteen, whichever is later. How we protect your information

Patient records are kept secure using administrative, physical and technical safeguards appropriate to their sensitivity. Access is limited to staff who need it to do their work, and our team is bound by confidentiality obligations. This website is served over an encrypted HTTPS connection.

Your rights

Under PHIPA you have the right to:

  • ask for access to the personal health information we hold about you
  • ask us to correct information you believe is inaccurate or incomplete
  • ask questions about our information practices, or withdraw a consent you previously gave
  • make a complaint about how your information has been handled
